The Nutritional Assessment Questionnaire (NAQ) is an assessment developed by Dr. Dicken Weatherby, N.D. that includes a 321 signs and symptoms questionnaire that goes over all the systems of the body including upper gastrointestinal, liver and gallbladder, small and large intestine, mineral and vitamin needs, sugar handling, essential fatty acids, adrenal, pituitary, and thyroid, along with male or female reproductive system. It will pinpoint where there is dysfunction and potential nutrient deficiencies based off your responses to the signs & symptoms questions.
